DMV Application
Once you feel confident you can pass the written test at the OMV. Print out this application,
https://public.powerdms.com/LADPSC/documents/348140
Take it and your DOT Physical to the OMV.
The OMV will need a copy of your DOT Physical. Google DOT Physical in your area to obtain one.
Don't forget to bring this copy to the OMV when taking the written permit test. Once you have your Louisiana Permit, we can start the BTW Training!

Step 2. Background Check (Location information available in the provided link) https://tsaenrollmentbyidemia.tsa.dhs.gov/locator...
Step 3. Take the written test at your local DMV (expresslane.org)
Step 4. Waiting for the background to clear. You'll get a letter letting you know when your background has cleared. Return that letter to the DMV office and they will add the endorsement to your CDL.
